  • Customized competency testing
  • National criminal history checks on all employees
  • Copies of all credentials including any state licensure or certifications
  • Verification of eligibility on state elder and child abuse registries
  • Copies of college degrees and/or certifications
  • Signed and completed 1-9 and copies of appropriate documents
  • Current physical exam records - annually
  • Current TB tests results - annually
  • Immunization record
  • Hepatitis B vaccination/immunity or signed waiver
  • OSHA Training on occupational exposure to bloodborne pathogens
  • Additional OSHA training on:
  • Transmission based precautions
  • Hepatitis B and HIV
  • Universal precautions
  • JCAHO approved facility safety training in:
    • Facility safe practices
    • Lifting and moving safely

  • JCAHO approved client care training in:
    • Patient confidentiality
    • Residents rights
    • Preventing patient falls

  • Drug screening available in accordance with state or client requirements
