- Clinical skills inventory
- Customized competency testing
- National criminal history checks on all employees
- Copies of all credentials including any state
licensure or certifications
- Verification of eligibility on state elder and child
abuse registries
- Copies of college degrees and/or certifications
- Signed and completed 1-9 and copies of appropriate
documents
- Current physical exam records - annually
- Current TB tests results - annually
- Immunization record
- Hepatitis B vaccination/immunity or signed waiver
- OSHA Training on occupational exposure to bloodborne
pathogens
- Additional OSHA training on:
- Transmission based precautions
- Hepatitis B and HIV
- Universal precautions
- JCAHO approved facility safety training in:
- Facility safe practices
- Lifting and moving safely

- JCAHO approved client care training in:
- Patient confidentiality
